Membership

Due to the sensitive nature of the content we distribute, the EPTA requires that you sign up for a free membership number. This number will allow you access to restricted information on the website, as well as important mailings. Below are the membership applications. If you have Adobe Reader you can open the Adobe version of the application, type in your information, then fax the information back to us. For those of you who do not have Adobe Reader we have also provided the application in Microsoft Word. Simply print out the application, fill in your information, and fax it back to us at 866-704-2253.
